How to Refer Us

At Texas Professional Hearing Center, we work with a large network of referring physicians and primary care providers in the greater Houston area and suburb communities. Currently, Medicare beneficiaries must obtain a physician referral prior to seeing an audiologist in order to have those services reimbursed by Medicare. To assist with your referral, please download and fill out the Patient Referral Form and fax to 281-420-8057.
